2008 Supplemental Benefits

Caremark offers you a variety of supplemental benefits, designed to assist you and your family beyond what our basic health and welfare plans provide.

Supplemental Life Insurance

You may purchase additional Employee Life Insurance for up to five times your benefit pay, in addition to your Basic Life Insurance. Your total life insurance, which is a combination of Basic and Supplemental coverage, cannot exceed $1,000,000. Visit page the 2008 Benefits Guide to learn more.

Spouse and Child Life Insurance

You can purchase life insurance for your spouse and dependent children through the Spouse and Child Life insurance options. Note: If you and your spouse are both benefit eligible employees of the Company, you cannot cover your spouse under Spouse Life Insurance. Only one of you may cover your eligible dependent child(ren) under Child Life Insurance. Visit the 2008 Benefits Guide to learn more.

Accidental Death and Dismemberment (AD&D) Insurance

Voluntary AD&D coverage provides 24-hour-a day protection from accidental death or dismemberment. Any AD&D benefits are in addition to benefits payable from Employee Life Insurance. Visit the 2008 Benefits Guide to learn more.

Short Term Disability (STD)

In the event of an injury or illness that requires you to be off work, your approved Short Term Disability benefits provide you with 70% of your base pay, with no monthly limit. Depending on the duration of your illness or injury, these benefits continue up through your 180th day of disability, at which time, you become eligible for Long Term Disability. Long Term Disability (LTD)

The LTD plan is designed to replace a percentage of your benefit pay in effect at the time of your disability. The elimination period for LTD benefits is 180 days. If you cannot perform the duties of your occupation, your LTD benefit replaces 50% of your benefit pay, up to a maximum of $10,000 a month for the first two years. Long Term Disability Buy-Up

In addition to the Company provided LTD, you are also eligible to elect the Long Term Disability Buy-Up coverage. The LTD Buy-Up Plan, provided by Unum, allows you to insure a greater percentage of your earnings, and enjoy discounted group rates only available through Company-sponsored plans. The Long Term Care benefit gives you and your extended family an opportunity to plan for all stages of your life. Cancer Protector Plan

Administered by AFLAC, the Cancer Protector Plan is designed to offset medical expenses related to cancer treatment. While your medical insurance will pay a portion of your medical expenses related to cancer treatment, there are other expenses such as deductibles, coinsurance, copays, and time off work, etc., to consider. Hospital Indemnity Plan

Personal Accident Indemnity Plan
